The Giallorossi took the lead early through Pier Luigi Simonetti and, after briefly being pegged back, stormed out into a 3-1 lead thanks to Devid Bouah and Ludovico D'Orazio - only to be hauled in by the Czech side, who scored in the 90th minute to win the game.
“We need to find our structure defensively,” coach Alberto De Rossi said afterwards.
“We did well for 60 minutes, we competed but we lacked quality in defence. The whole team needs to defend better. When the opponents push the tempo, drive forward and move the ball into space we find ourselves in trouble.”
Elsewhere in Group E, Real Madrid beat CSKA Moscow 4-1, leaving Roma bottom of the fledgling group table with no points.
