The 18-man selection, chosen by UEFA's technical observers, is meant to highlight the top performers over the course of the entire competition.
Goalkeeper Alisson enjoyed an impressive debut campaign in the Champions League, not conceding a goal at the Stadio Olimpico until the semi-final stage. UEFA's observers paid particular tribute to his performance away to Shakhtar Donetsk, in the last-16, as a reason for his conclusion.
Dzeko, meanwhile, scored three goals in the group stage - before scoring in five successive knockout stage games to help drive the Giallorossi to the last four.
"A brilliant volleyed finish in the group stage against Chelsea was a highlight as he led the line and his team to the semi-finals, scoring eight goals along the way," the observers said.
Former Roma winger Mohamed Salah was also named in the squad.
Winners Real Madrid had the most representatives, with eight.
